A Vuelta Health staff member will contact you to schedule your Meet & Greet appointment.


Please make sure to bring copies of your last 3 Primary Care visit notes and a complete list of all medications you are currently taking.


Meet & Greet appointments are typically 30-45 minutes, this time is meant to discuss goals of care, determine the membership plan that will best serve you and ensure a good fit for patient and provider. Medical Record Review and Membership Agreement discussion will take place during this time.


Meet & Greet appointment is not a commitment to membership by the patient, nor a guarantee of services by the provider.
