What is Direct Primary Care?

Direct Primary Care (DPC) is a healthcare model where patients pay a simple monthly or yearly membership fee directly to their primary care provider for routine and preventive care. Instead of billing insurance for every office visit, test, or message, the provider and patient work directly with one another. Most DPC practices include services like same- or next-day appointments, longer visits, chronic disease management, preventive care, basic procedures, and direct communication by phone, text, or patient portal — often with transparent pricing and no copays.

The reason many patients and providers are moving toward DPC is because traditional insurance-based primary care in the U.S. has become increasingly expensive, rushed, and administratively complicated. Providers often have to see very high numbers of patients each day to keep up with insurance billing requirements, which can lead to shorter appointments, burnout, long wait times, and patients feeling unheard or disconnected from their care.

DPC attempts to remove many of those barriers by simplifying the relationship between provider and patient. Without insurance companies dictating visit structures, prior authorizations, or reimbursement rules for routine primary care, providers can spend more time with each patient and focus on prevention, education, and long-term wellness instead of volume-driven care. Many practices intentionally keep smaller patient panels so patients can build ongoing relationships with a provider who truly knows them.

From a system perspective, DPC is also growing because many Americans — especially uninsured, underinsured, self-employed, and small business employees — are frustrated with rising healthcare costs and confusing insurance plans. DPC offers predictable pricing and easier access to care, which can help patients address issues earlier before they become expensive emergencies.

It’s important to note that DPC is not health insurance. Patients still typically carry insurance for major medical events, hospitalizations, surgeries, or specialty care. Instead, DPC is designed to improve access to affordable, relationship-based primary care — the part of healthcare most people use most often.
